The Sutton Palliative Care Coordination Hub is recognised as a system-wide resource that can support the transformational change that is taking place across Sutton's health and social care system. It serves as a single point of access for people aged 18 and over who are likely to die within 12 months. It provides a single referral point for care, coordination and delivery for professionals, individuals, families and carers that will be managed by nurse specialists who are experts in palliative care and end of life.
The team supports early identification and advance care planning by proactively identifying patients who should be receiving palliative care and facilitate timely discharge from hospital through coordination of commissioned services
Main duties of the job
The Palliative Care post holder will be responsible for assessing, planning and utilising skilled, high quality, evidence based nursing care for patients with palliative care needs, including the terminal phase, and for supporting the family prior to and immediately after death.
The post holder will be responsible for anticipating and responding to changing patient needs whilst maintaining their independence, privacy and dignity.

Education/Qualifications
Essential criteria
1. Up to date clinical registration - RN, AHP, Paramedic
2. Evidence of continuous professional practice and education
Desirable criteria
3. Relevant post registration course in palliative care with experience of working in an EOLC specialty
4. Advanced communication course
5. Independent prescribing qualification
Experience
Essential criteria
6. Significant experience working with older people, those with dementia and frailty and those entering the final phase of life
7. Experience in undertaking holistic assessments
8. Experience in managing the needs of complex patients
9. Experience of working in community
Desirable criteria
10. Experience in supervising, mentoring and appraising staff
11. Recent experience in teaching and assessing in clinical practice
Skills and Abilities
Essential criteria
12. Clear understanding of current national and local agenda relating to delivery of community care
13. Sound understanding of patient management for older people and people living with a long term condition
14. Ability to demonstrate evidence based clinical practice
15. Awareness of professional issues and national agenda regarding integration of health and social care
16. Knowledge of the National Service Framework for Older People and its application to practice
17. Knowledge of the National Service Framework for Long Term Conditions and its application to practice
