Mental Health Social Worker – Oxford Health NHS Foundation Trust
We are seeking a qualified Social Worker to join the Adult Mental Health Team in North & West. You will provide statutory social care expertise, ensuring comprehensive assessment, support planning and multidisciplinary collaboration for service users with complex mental health needs.
Responsibilities
* Identify and address barriers to promoting a social model of mental disorder in partnership with service users, families and the multidisciplinary AMHT.
* Provide expertise regarding the application of the Mental Capacity Act and Mental Health Act.
* Articulate the social perspective of mental health within the multidisciplinary team, ensuring social care needs remain a high priority and profile.
* Conduct Care Act or s.117 aftercare eligibility assessments, particularly for individuals presenting with highly complex needs.
* Demonstrate professional leadership and skill in situations characterised by high levels of social, family and interpersonal complexity and risk.
* Formulate social care support plans following the requirements of the Care Act 2014 and s.117 aftercare eligibility using the required local authority processes.
* Assess carer’s needs and implement an appropriate support plan.
