1 Month Contract With A Local Authority Job Purpose: As a Social Worker, you will provide professional support and intervention to adults with care and support needs, helping them maintain independence, safeguard their well-being, and improve their quality of life. You will work in partnership with individuals, families, carers, and multi-agency teams to deliver effective, person-centred outcomes. Key Responsibilities: Undertake strengths-based assessments of individuals’ care and support needs in line with the Care Act 2014. Develop, implement, and review care and support plans tailored to individual needs and goals. Safeguard adults at risk by identifying and responding to concerns in accordance with safeguarding procedures. Work collaboratively with health, housing, voluntary sector, and community services to ensure holistic support. Provide information, advice, and advocacy to enable informed decision-making. Maintain accurate and timely case records using electronic case management systems. Participate in multidisciplinary meetings, case conferences, and reviews. Manage a varied caseload, prioritising effectively to meet statutory responsibilities and deadlines. Promote and uphold the principles of dignity, choice, and independence. Keep up to date with legislation, policy, and best practice in adult social care. Requirements: Degree in Social Work (or equivalent) Registration with Social Work England Understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks Experience working with adults in a social care or health setting Strong communication, assessment, and case management skills Ability to work independently and as part of a team Commitment to anti-discriminatory practice and service user empowerment Additional information Enhanced DBS Is required Working Hours: 36 Per Week
