Discharging an individual on to the right care pathway when they no longer need to remain in a hospital or community bed requires a whole system approach. Every local health and care system has a Care Transfer Hub where (physically and virtually) relevant community services across
sectors (e.g. health, social care, housing, voluntary sector) are linked together.
The hub coordinates care for people who require care and support after discharge from hospital, and any support for unpaid carers providing care. This covers any patient discharged from hospital or community beds regardless of the area they are resident in. The objective for the team is to lead discharge planning for patients leaving hospital on all pathways and ensuring that as many patients as possible get the support they need to go home.
The Care Transfer Hub should be involved with complex patient discharge from early in admission to plan for and mitigate problems.
Main duties of the job
Supervises work of others in the team or MDT and provides regular clinical supervision of staff and students.
Plans provision of care for discharge of patients. May organise staff, e.g. make short-term adjustments to rotas or work activities for a shift such as discharge planning.
Works independently within organisational and professional guidelines, deciding when to escalate to a senior clinician (e.g. delays to patient discharge).
Maintain flexibility in the role to adapt to changing professional or service needs.
Undertake clinical duties to maintain professional competency, authority and credibility.
Lead on developing specialist assessments for patients with complex needs relating to discharge planning.
Support safe and appropriate discharges through a “Home First” approach, actively working to reduce length of stay.
Provide specialised advice relating to discharge.
Assess, plan, implement and evaluate clinical care of patients.
Order and co-ordinate the timely delivery of equipment required for the discharge
Liaise and co-ordinate with other agencies involved in the discharge plan (eg Voluntary sector, HSRT)
Liaise with families, partners and ward staff to ensure accurate, up-to-date records in systems (e.g. Connect, Digital Discharge Tool, SystmOne).

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being part of an organisational change within the Care Transfer Hub. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and the management of organisational change.
Please note all existing NHS staff will only be considered on a secondment basis. This must first be agreed with your current line manager.

Education and Qualifications
Essential criteria
1. Valid NMC registration
2. First Level degree or evidence of working towards one
3. To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998.
4. Management and Leadership qualification/certificate or equivalent at Post Graduate level or willingness to work towards
5. Evidence of continuous personal and professional development
Experience
Essential criteria
6. Evidence of multi-disciplinary working.
7. Management and Leadership experience including managing attendance, recruitment and selection
8. Broad post registration experience and knowledge of system discharge gained through Primary or Secondary Care.
Desirable criteria
9. Evidence of implementing and managing complex change in a multi-disciplinary area
10. Experience of working in challenging performance programmes
Skills and Knowledge
Essential criteria
11. Can prioritise and manage own workload when faced with conflicting pressures
12. Demonstrates a high standard of written and verbal communication with the ability to present and receive complex information and communicate effectively with all grads of staff, patients and relatives
13. Knowledge of Discharge to Assess and Home First principles
14. IT skills including Datix and knowledge of STH clinical IT systems

15. Knowledge of current local and national policies, particularly those related to discharge
