We are seeking an enthusiastic and highly motivated Senior Occupational Therapist to work within our Integrated Neighbourhood Teams (INTs).

The INTs are innovative and dynamic, providing active rehabilitation to patients in their homes to maximize functional potential and support independence. Our therapists support hospital discharges, admission avoidance, and long-term self-management, enabling patients to live better lives at home.
You will work in a friendly, supportive environment alongside other Therapists, Assistant Practitioners, Paramedics, Nurses, and other healthcare professionals.
We offer a flexible service, operating extended hours 7 days a week.

Key Responsibilities:
1. Initial holistic assessment, implementation, and evaluation of care programmes for patients with complex needs.
2. Use clinical reasoning to assess, plan, implement, and evaluate patient-centered interventions using evidence-based practice.
3. Manage your own caseload proactively.
4. Coordinate work within the team for efficient service delivery.
5. Develop strong relationships with service users, carers, and families to maximize quality of life and independence.
6. Facilitate referrals to other services based on assessments.
7. Assess and arrange equipment provision within scope of practice, reporting issues.
8. Support the development of team members and students.
9. Maintain accurate and legal records.
10. Participate in training and development activities.
11. Identify and minimize risks to self, colleagues, and service users.
