As a Support Worker, you will play a key role in delivering person-centred, high-quality support that promotes wellbeing, independence, dignity, and inclusion.
Key Roles and Responsibilities:
1. Deliver high-quality, person-centred support tailored to the individual needs, goals, and aspirations of the people you support.
2. Support individuals with daily living, personal care, tenancy sustainment, emotional wellbeing, and social inclusion where required.
3. Contribute to and implement personalised support plans, participating in regular reviews, goal setting, and risk assessments.
