In our quest to deliver patient-focused quality care we are looking to recruit a highly motivated, innovative & enthusiastic Physical Health & Wellbeing Nurse Lead (RGN, RMN or RNLD) to join our forward thinking, friendly & expanding Health & Justice team split between Park Royal Low Secure Services & ReFoCuS Community Forensic Team.

Our healthcare services work to a 7-day week working model, to ensure our patient's healthcare is always managed & maintained to the highest of standards.

Our motto is “Caring NOT Judging” so by working in partnership with the criminal justice system & other agencies, we provide responsive, dependable & high-quality primary care & mental health services to those who may have offended & those at risk of offending in the communities we serve to improve their health status & to encourage social inclusion.


Responsibilities:

* Oversee & undertake physical health checks & offer health promotion support to complex patients with severe & enduring mental health problems who are under the care of the low secure forensic services & community forensic team.
* Provide advice, supporting clinical & social care interventions for service users whilst holding a caseload of community forensic patients.
* Supervise & support the teams to deliver high standards of physical care to patients with long term conditions in accordance with up to date, evidence-based professional practice subject to local & Trust policies & to provide leadership to the nursing teams within the services.
* Lead on preparation & delivery of health promotion initiatives linking in with Trust resources, local business & healthcare providers.
* Support the administration and facilitation of service user forums & feedback relating to physical health and health promotion initiatives.

Key Responsibilities:

* Advise and contribute to the delivery of high-quality nursing care for patients with physical health issues within the forensic service.
* Lead on the development and delivery of high-quality nursing care plans for patients with our services and advise on physical healthcare including diet, weight gain, exercise, lifestyle, sexual health, smoking and medication side effect monitoring.
* Develop, co-ordinate and contribute to the delivery of physical health and well-being training programmes and educational materials for staff in collaboration with senior members of the services.
* To be conversant with, Adult Safeguarding, NSF, Mental Health Act, Mental Capacity Act and key national Mental Health policies.
