The Powys Living Well Service is seeking a dedicated and compassionate Assistant Practitioner (Band 4) to support individuals living with persistent pain, fatigue, and weight management. You will contribute to service improvement projects, helping to shape and enhance care delivery for optimal outcomes.
Main duties of the job
As an Assistant Practitioner, you will manage your workload independently under supervision, supporting individuals with long-term conditions. Your responsibilities include providing evidence-based care, assisting in assessments, and supporting rehabilitation and self-management programs.
Key Responsibilities:
1. Delivering Care
o Support initial consultations, implement, and modify care plans.
o Assist in rehabilitation sessions and weight management interventions.
o Promote person-centred care to enhance health and independence.
2. Monitoring Progress
o Recognize and respond to changes in physical or mental health.
o Provide feedback and contribute to assessments.
o Ensure safe, high-quality care and escalate concerns as needed.
3. Mentoring Junior Staff
o Support and supervise healthcare assistants and students.
o Provide guidance and foster a positive learning environment.
o Uphold professional standards and best practices.

Person Specification
Qualifications: Essential: Healthcare certificate (Level 4 CQFW Credits), NVQ Level 3, A-Levels, BTEC, or equivalent experience. Desirable: Relevant courses/study days.
Experience: Essential: Recent clinical experience in a healthcare setting. Desirable: Experience in the relevant specialty.
Aptitudes & Abilities: Essential: Teamwork, independence, decision-making, work under pressure, communication skills, proficiency in Microsoft Office. Desirable: Diffusing difficult situations.
Values: Essential: Alignment with Health Board values. Desirable: Understanding of weight stigma.
Other: Essential: Flexibility, understanding of health and safety, ability to travel. Desirable: Welsh language skills.
