About The Role
This is an exciting time to work in the Adults Learning Disabilities and Mental Health Placements and Brokerage Team in Hounslow.
A new opportunity has arisen for an organised individual who is eager to ensure the best value and care is provided for vulnerable adults in the borough. You will be part of an exciting pilot project delivering a shared lives service to vulnerable residents of Hounslow.
* You will be working with partners to effectively deliver the service.
* To support the priorities of developing and implementing the shared lives in house provision to meet the needs of the Learning Disabilities and Mental Health community, and potentially the expansion of the pilot across care groups
* You will support in putting in place processes and practices to prevent/reduce the requirement to place clients with LD/MH specific needs out of borough. By ensuring that you maximise the shared lives programme
* You will have the ability to achieve positive change through a systems approach to problem solving.
* As a placement team, our aim is to successfully place residents in good quality care provision ensuring outcomes are met for our residents, facilitating their progression to as independent life as possible. If successful in your application, you will be at the forefront of the Shared Lives pilot project, demonstrating its value and success.
* You will have the opportunity to develop your skills in service development, review and evaluation.
About The Team You’ll Be Working In
Hounslow have an ethos of recovery model working and the team are key to the progression of clients through this process. We contract manage those providers that are based within the borough, using audits, unannounced visits, contract monitoring meetings, and quality alert investigation. We have developed a positive working relationship with our providers and continue to develop the market. We are very excited about how this pilot project can potentially change the outcomes for people that use our services.
We joint work closely with our ICB partners, the social work teams and others within commissioning.
About You
* Passionate about ensuring that vulnerable clients receive the level of care/support that they require to enable as independent life as possible.
* To be solutions focused and open to different options and approaches to resolving issues.
* Ability to communicate appropriately with different audiences both within the local authority and with partner agencies, including the ability to non-judgementally challenge service delivery.
* Assessment of the suitability of the accommodation.
* Match potential clients with shared lives providers
* You come with a passion to ensure that the right services are delivered to the right people, in the right way. That independence and potential can be realised, through the least restrictive solution to placement.
* You will have a pragmatic attitude to problem solving, be flexible and adaptable to a changing picture.
* Enhanced DBS check is required.
