Early Years Practitioner

We have an exciting opportunity in the Sunderland 0‑19 Service working within our Health Visiting Teams for an Early Years Practitioner.
The successful applicants will be based in Sunderland working within our teams to deliver care across the Sunderland locality.
Main duties of the job
The Early Years Practitioner is a member of the 0‑19 Integrated Public Health Service (IPHS), working within the policies and procedures relating to the 0‑19 service.
The post holder will work closely with the SCPHN within a skill mixed team to support the delivery of the 0‑19 Service Specification and Healthy Child Programme (HCP).
The aim of the service is to provide universal access to children, young people and families to the Healthy Child Programme and facilitate early identification of additional and/or complex needs.
The post holder will contribute to the Public Health Agenda, support the promotion of health and reduction in health inequalities, and work in partnership with key partners and agencies to meet local and national agendas.

Job responsibilities
As part of the Health Visiting Teams, you will:
* Deliver and promote the Healthy Child Programme to children, young people and families.
* Identify early and complex needs and refer to appropriate specialist services.
* Work collaboratively with statutory and voluntary partners to support families.
* Maintain clear and confidential records of care and support activities.
Person Specification
Qualifications
* NVQ Level 3 Qualification or equivalent.
* Experience of working with and supporting parents and families.
* Experience of facilitating group work.
* Experience working with a range of statutory and voluntary organisations.
Experience
* Ability to handle sensitive data with confidentiality.
* Experience working in a healthcare setting.
Knowledge
* Knowledge of child development.

Salary
£28,392 to £31,157 a year (pro rata)
