We are seeking a dedicated and compassionate Advanced Nurse Practitioner to join our friendly and supportive clinical team at Oakview Medical Practice, located in the Vale Centre for Health & Care in Alexandria and at our satellite Practice, the Renton Integrated Healthy Living Centre. The ideal candidate will possess advanced clinical skills and a strong commitment to providing high-quality patient care. As an ANP, you will play a vital role in diagnosing and treating patients, promoting health education, and collaborating with other healthcare professionals to ensure comprehensive care.
Our team currently consists of 10 GP Partners, 1 salaried GP, 3 Practice Nurses, a Practice Pharmacist and an extensive administrative team. We also have working within the practice, 3 PCIP Pharmacist, 2 Phlebotomists, a Physiotherapist, a Community Link Worker, and we have access to a health board treatment room service in the building. We pride ourselves on providing high quality services, tailored to meet the needs of our 9,400 patients.
Duties
1. Triage patients presenting to the practice and prioritise care.
2. Conduct thorough patient assessments, including medical histories and physical examinations.
3. Diagnose and manage acute and chronic illnesses, including sexual health, developing appropriate treatment plans.
4. Apply clinical reasoning to manage undifferentiated and undiagnosed presentations.
5. Prescribe medications and therapies in accordance with established protocols.
6. Review and de-prescribe medications where appropriate.
7. Order and interpret investigations such as blood tests.
8. Provide health education and counselling to patients and their families on disease prevention and health maintenance.
9. Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
10. Maintain accurate patient records and documentation in compliance with regulatory standards.
11. Stay updated on the latest medical practices, guidelines, and advancements in healthcare.
12. Participate in clinical supervision mentorship.
13. Contribute to professional development activities within the team.
14. Participate in audits and quality improvement projects.
