Liquid Personnel is recruiting Occupational Therapists for its clients Integrated Discharge Service; Occupational Therapy & Sensory Service Team located at Hackney.
Join our client's dynamic Hospital Team delivering high-impact occupational therapy interventions that promote independence and support safe, person-centred hospital discharges. This is a hybrid role with opportunities to work across London hospitals and in the community.
What will your responsibilities be?
Assess Hackney residents in hospitals across London for safe discharge planning
Collaborate with multi-disciplinary teams and attend ward meetings
Support residents with complex needs, including LD, MH, dementia, and NRPF
Conduct community reviews and Care Act assessments
Promote co-production and independence in daily livingExtras:

To be successful in this role you must have,
HCPC-registered Occupational Therapist
Experience in hospital or social care settings
Strong assessment and Care Act review skills
Knowledge of safeguarding, MCA, and discharge planning
