Are you driven to make a real difference in people’s lives from patient own homes? We’re looking for a compassionate and proactive Band 6 Deputy Sister/Charge Nurse to join our well-established York Community Response Team.
Responsibilities
* Deliver high-quality holistic care to prevent unnecessary hospital admissions, alongside specialist nursing interventions
* Support timely discharges and promote recovery at home
* Collaborate with a multidisciplinary team including Physiotherapists, Occupational Therapists, Nurses, and Support Workers
* Engage patients and families using health coaching techniques to set realistic, empowering goals

* Location: White Cross Rehabilitation Unit, York, YO31 8JR
* Band: 6
* Salary: £38,682 to £46,580 per annum / pro rata
* Contract: Permanent
* Working pattern: Full-time, Part-time, Flexible
Essential qualifications and experience
* Education, Qualifications and Training
o Registered Nurse
o Recognised teaching / assessing qualification
o Evidence of continuing professional development and implementation in clinical practice in the community within the last 12 months
* Experience and knowledge
o At least 3 years’ experience at Band 5 or equivalent
o Insight into community rehabilitation and re-ablement issues
o Insight into current nursing/professional issues
* Skills and attributes
o Excellent organisational and prioritising skills
o Effective communication with patients and the multi-disciplinary team
o Reliability, adaptability to change, and ability to work on own initiative or as part of a team
o Good verbal and written communication skills including telephone manner
o Ability to teach, supervise and assess registered/unregistered staff
