We are looking for a dedicated and proactive NMC registered Practice Nursing Associate to join our fun and friendly primary care team. As a key member of our clinical team, you will support the delivery of a wide range of nursing services within a primary care setting. Youll work closely with GPs, practice nurses, and the wider multidisciplinary team to ensure patients receive safe, effective, and compassionate care.

Main duties of the job
Deliver highquality nursing care in line with practice protocols
Undertake cervical screening, immunisations, wound care, and longterm condition support (as appropriate to role and competency)

* Deliverhigh quality, compassionate care while undertaking specific clinical and caretasks under the direction of a registered nurse (or other registered careprofessional) with a focus on promoting good health and independence
* Providehigh-quality holistic and person-centred care to individuals
* Provideroutine care to patients as required in accordance with clinical basedevidence, NICE and the NSF
* Providewound care, dressings and other clinical tasks as required
* Workas part of the MDT to provide and monitor care, under direct or indirectsupervision and ensure delivery of nursing associate duties
* Improvethe safety and quality of care at every opportunity
* Contributeto the delivery of integrated care
* Workwith a supervisor to take responsibilityfor developing own clinical competence, leadership and reflective practiceskills within the workplace and enable you to deal effectively with thedifficult issues that people present
* Supportregistered nurses to enable them to be able to focus on the more complexclinical care
* Developrelationships across the MDT to support the integration of the role acrosshealth and social care including primary care, secondary care and mental health
* Maintainaccurate and contemporaneous patient health records ensuring that clinical datais appropriately recorded with SNOMED codes
* Performand record clinical observations such as blood pressure, temperature,respirations and pulse including appropriate coding
* Afterundertaking additional training, provide flu vaccinations, ECGs andvenepuncture and other relevant clinical tasks as required and in line with thecompetencies of the role
* Promotehealth and wellbeing to all patients, for example undertaking the NHS healthcheck
* Providewound care (ulcer/Doppler etc.) to patients
* Providesupport during minor operations as required
* Advisepatients on general healthcare and promote self-management where appropriate,including signposting patients to personalised care colleagues and localcommunity and voluntary sector services
* Communicateproactively and effectively with all MDT colleagues, attending and contributingto meetings as required
* Beaware of duties and responsibilities regarding current legislation and adhereto practice policies and procedures on Safeguarding Adults and SafeguardingChildren
* Enhanceown performance through continuous professional development, imparting ownknowledge and behaviours to meet the needs of the service
* Contributeto and embrace the spectrum of clinical governance
* Attenda formal appraisal with your manager at least every 12 months. Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed
* Contributetowards public health and screening programmes, including immunisations andvaccinations
* Supportand deliver health promotion and wellbeing programmes including anyopportunistic delivery
* Maintain a clean, tidy, effective working area
* Recogniseboundaries of practice and know when and to whom patients should be referred
* Demonstrateinitiative and be creative in finding solutions to problems
* Supportin the management of patient complaints when requested to do so and participatein the identification of any necessary learning brought about through incidentsand near-miss events
* Undertakeall mandatory training and induction programme.

Qualifications
* Meets the specific qualification and training requirements as specified in the Nursing Midwifery Standards of proficiency by having undertaken and completed the two-year Foundation Degree delivered by a Nursing and Midwifery Council (NMC) approved provider.
* Previous experience in a primary care environment is essential.
