We are a countywide service providing a highly skilled and supportive Home Treatment team. We build relationships, problem solve, listen, assess multiple care needs, direct plans of care, provide signposting, advice, education, and support to individuals living with dementia and their families, carers, and loved ones.
We seek a highly motivated individual passionate about working with people living with dementia and promoting the concept of living well with dementia. You should have excellent communication skills, be enthusiastic, flexible, and demonstrate an understanding of the complexities involved in this specialist area.
As part of our team, you will work autonomously in a varied role that offers opportunities to demonstrate and develop your skills. You will be supported by our multidisciplinary team and collaborate with wider health and social care communities, including general practice, care homes, social care, admiral nurses, and third-sector services such as the Alzheimer’s Society.
This community-based role requires frequent travel within a wide geographical area to support service delivery.
We welcome applicants from both experienced and newly qualified nurses.
Main duties of the job
* Manage an identified caseload under supervision from a Band 6 Nurse, including specialist assessment, care planning with service users and carers, and evaluation of treatment plans.
* Participate in setting quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance, practice, and policy.
* Assess patients' individual care needs, develop, implement, and evaluate care programs.
* Deliver interventions according to the care plan and pathway guidance, including pharmacological and psychologically informed interventions.
* Complete and contribute to risk assessments and risk management plans, promoting positive risk-taking to enhance quality of life and independence.
* Maintain effective reporting systems by observing and documenting patient conditions verbally and in writing.
* Utilize outcome measures to review intervention effectiveness during care planning and evaluation.
* Support training of students, staff, carers, and service users.
* Participate in service development to ensure clinical governance is actively implemented at the team level.
