Job summary

An exciting new opportunity has arisen to join the Stop Smoking Service team based at Forsyth House, supporting the delivery of a high-quality smoking cessation service across Darlington.

This role will play a key part in enhancing and maximising opportunities to provide Very Brief Advice (VBA) to people who smoke across all primary healthcare settings. By working collaboratively with healthcare professionals and community partners, the postholder will help increase engagement with stop smoking support and contribute to reducing smoking prevalence locally.


Main duties of the job

to support the Stop Smoking Service team, based at Forsyth House, Darlington, in delivering a smoking cessation service to the local population. The role will enhance and maximise opportunities to deliver Very Brief Advice (VBA) to people who smoke across all primary healthcare settings. This will contribute to key outcomes of the service and necessary support to reduce smoking prevalence in Darlington, with a particular focus on targeting and engaging those most likely to benefit from stop smoking assistance. The role will involve some evening and weekend working, promoting the service within the community in and around Darlington.


About us

Primary Healthcare Darlington is a GP Federation, based centrally in Darlington, which supports 11 GP practices in Darlington to provide the best possible healthcare services.

We also provided an extended GP Access Service, a Community Sexual Health Service, private medicals and primary care services and an integrated service to the Care Homes within Darlington.

Job responsibilities

Key responsibilities:

To be the key liaison between the Stop Smoking Service and the GP Practices, as well as other primary health care settings, acting as a Single Point of Contact (SPOC) for any queries or questions and provide additional support.o be able to fully explain how the Service operates, including how to refer to the service and what stop smoking support aids are available to support them This includes undertaking relevant training on NRT, vapes and Pharmacotherapy

Supporting clients within Darlington to refer to the stop smoking service or begin the behaviour change journey

Providing continuous support to people to make the decision to make a quit attempt or to try again.

Provide Mini awareness sessions/drop ins from any primary health care setting waiting areas, as appropriate, promoting the Service and outlining the products available to support smokers quit and offer links to relapse prevention.

Working with Primary Care colleagues the champion will target their support to people living within the borough, with long term conditions exacerbated by smoking.

Be responsible for requesting and coordinating the supply of medication and/or quit aids

Have access to and be competent in using System1 to be able to target areas of borough with the highest smoking prevalence to support them to refer into the service.

Participate in community engagement activities across Darlington to promote smoking cessation services

Recording information within the client's medical records (SystmOne) and other data collecting systems

Ensuring messages and information are passed on to relevant members of staff or outside agencies

Running reports

Telephone

Have working knowledge of the telephone system, during and after hours.

Provide an efficient system of answering the telephone using a courteous and friendly manner

Liaising with healthcare professionals and other professional bodies

Attending community events and promoting the service

Training

The post holder will be suitably trained to a nationally recognised standard of training such as that provided by NCSCT, and can demonstrate that they are qualified and competent to deliver the Service, and be trained to,

Promote MECC (Making Every Contact Count) by making the most of an opportunity to raise awareness, share knowledge and get a person thinking about making changes to improve their health, wellbeing and behaviours.

Deliver VBA and /or interventions, that can be 'opportunistic', taking very little time and can be implemented anywhere on the intervention continuum.

Deliver brief interventions recognising that many people can benefit from being given appropriate information at the right time particularly relating to the harms and risks associated with tobacco and smoking and other risks such as second-hand smoke.

Provide support to stop smoking through encouragement and behavioural change support

Run data reports and requests on system one

Health and Safety Responsibility:

It is the responsibility of the individual to work in compliance with all current health and safety legislation and the organisation Health & Safety Policy and to attend any training requirements both statutory and mandatory in line with the ` legal responsibility to comply with the Health & Safety and Welfare at Work Act 1974.
