Community Support Worker

Main Duties
Support service users to access the care and resources needed to address both their mental and physical health needs, promoting recovery and overall wellbeing. All duties must be carried out in line with Trust values and the NHS Constitution, maintaining high standards of professionalism at all times.
Support the management and care of service users under supervision, contributing to ongoing assessment, observation, and risk assessment, and providing timely feedback to senior staff.
Job Responsibilities
Assist with activities of daily living (meal planning, shopping, budgeting).
Support access to employment, housing, and benefits.
Accompany service users to appointments.
Undertake physical health care including:
* Complete physical health checks (e.g., blood pressure, pulse, weight, BMI, ECG where trained).
* Document and report results appropriately.
* Promote healthy lifestyles such as diet, exercise, and smoking cessation.
Assist in care planning and delivery, including:
* Participate in care assessments and reviews under supervision.
* Support low-level group interventions (relaxation, health promotion, sleep hygiene).
* Promote concordance with medication and treatment.
Maintain accurate and timely documentation, including electronic records, recording contacts and interventions, and reporting incidents in line with Trust policy.
Attend and contribute to team meetings (referral, review, and business meetings).
Work collaboratively within the multidisciplinary team and with external agencies.
Build professional, respectful relationships with service users, supporting them to identify needs, set goals, and engage in their care.
Respond appropriately to queries from patients, carers, and professionals, including complex or sensitive issues.
Support safeguarding, complaints, and incident investigation processes.
Driving or having access to a vehicle is required.
Person Specification
Skills and Experience
* Basic understanding of the nature of mental health conditions and how it can affect people’s day‑to‑day life.
* Understanding of the role of the Support Worker.
* Understanding of working with people with mental health conditions and an appreciation of vulnerability.
* Previous experience working in the NHS or health care sector.
* Experience working in the Mental Health environment.
Additional Qualities
* Current driving licence and access to a vehicle for work purposes.
* Ability to travel across sites and across Trust boundaries to attend meetings, etc.
Education / Qualification
* NVQ3 in Health or Social Care or equivalent experience.
* Good written and spoken English.
