About the Role
The responsibility of a Clinical Nurse Specialist with the Better Breathing Hawkes Bay Service is to assess patients for suitability to attend and participate in pulmonary and heart failure rehabilitation programmes. This role requires; a comprehensive initial assessment for referred patients in a clinic setting, and the assessment of Community Oxygen Therapy patients in their home environment, participating in the development and delivery of rehabilitation programmes, review of systems and processes to ensure service delivery is based on national and international best practice, monitoring service outcomes, both clinical and non clinical, supporting the Wairoa and Central Hawkes Bay providers with the delivery of rehabilitation programmes and education of patients, whanau and other health professionals.
The role requires you to foster and maintain relationships and links across the health sectors (Medical and Nursing teams, Community Health providers, patients and their whanau). The work is varied, rewarding and interesting. If you have not previously worked in the community environment, this a wonderful opportunity to support the team to provide quality care in a variety of settings.
About the Team/Service/ Location
The wider Community Nursing Service teams consist of the District Nursing teams in Napier and Hastings, the Continence, Ostomy and the Wound and Community Clinical Nurse Specialist teams,, as well as the Better Breathing Hawkes Bay Service (providing respiratory and heart failure rehabilitation and Community Oxygen Therapy services),
The roles within the wider Community Nursing Service teams includes Clinical Nurse Specialists, Associate Clinical Nurse Manager, Nurse Educator, Clinical Nurse Coordinators, District Nurses and Registered Nurses, Health Care Assistants, and a Community Kaiawhina, along with administration team support. Bay Fallen Soldiers Memorial Hospital Referrals are received from the primary, secondary and tertiary health sectors.

About you
You will need the following key attributes and skills:
* Current registration with Nursing Council of New Zealand.
* 5 years post graduate medical nursing experience
* Recent experience within a clinical setting
* Full unendorsed drivers licence
* Reliable and trustworthy
* Previous experience working within a team
* Ability to work autonomously
* Experience in a leadership role
