Advanced Nurse Practitioner –

Palliative Care

An exciting opportunity has arisen for an experienced Advanced Nurse Practitioner (ANP) to join our palliative care team at Woking & Sam Beare Hospice and Wellbeing Care, based in our bespoke facility in Goldsworth Park, Woking. This is a pivotal time for the Hospice as we develop new models of care, and we are seeking an ANP with extensive clinical experience and proven leadership skills to help shape the future of palliative and end of life care across Northwest Surrey.

As an integral member of our multidisciplinary team, you will work within the medical team, report to the Consultants in Palliative Care, and collaborate closely with our nursing teams. You will deliver expert, holistic care across our Inpatient Unit, Community Services, Outpatient Clinics, and the Wellbeing Centre, ensuring seamless care coordination and high standards of symptom management. Your clinical expertise will be matched by your ability to communicate effectively, work autonomously, and lead with compassion and innovation.

We are a patient-centred charity that delivers palliative and end of life care to those with advanced life-limiting illnesses, who live in Northwest Surrey. We care for patients and provide support to families and carers in the community as well as in our In-Patient Unit and Well Being Centre at the Hospice.

Clinical Colleague

Key Responsibilities:

1. Lead on complex symptom management, prescribing, and advance care planning.
2. Undertake autonomous assessments, diagnoses, and treatment planning.
3. Provide urgent clinical support in the community, including rapid response visits and crisis intervention.
4. Mentor junior staff and contribute to strategic service development, education, and governance.
5. Participate in a 1:4 or 1:5 on-call rota, supporting both inpatient and community services.
6. Support service innovation and contribute to audit, research, and policy development.
7. Work collaboratively with NHS partners and local healthcare providers to ensure continuity and integration of care.

8. A registered nurse with independent prescribing qualifications,
9. A passionate and skilled ANP with a strong background in palliative and end of life care.
10. Excellent clinical assessment and decision-making skills.
11. Outstanding communication, interpersonal, and organisational abilities.
12. Proven leadership and mentoring capabilities.
13. A flexible, proactive, self-motivated approach and commitment to continuous learning and reflective practice.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
