We are seeking a proactive and compassionate care coordinator to join our Primary Care Network (PCN), supporting residents across care homes. This role is central to improving the quality, continuity, and coordination of care for some of our most vulnerable patients. Minimum 20 hours per week, possibly Tuesday - Friday, this could be flexible for the right applicant.
Main duties of the job
The care home team manages over 500 beds, and the team work in collaboration with the surgeries in the network to deal with this caseload. Care coordinator work closely with GPs and other Primary Care Professionals within the PCN making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed. Our aim is always to provide a more joined up and coordinated care journey for patients. Acting as a single point of contact for care home patients you will play an important role within our PCN to proactively identify and work with people, including the frail, elderly, and those with multiple long term physical, mental health conditions and learning disabilities to provide coordination and navigation of care and support across health and care services.
Job responsibilities
* Act as a central point of contact for care home residents within the PCN
* Coordinate care plans, reviews, and follow ups for residents
* Support delivery of structured medication reviews and health checks
* Liaise with GPs, nurses, pharmacists, social care and external providers
* Monitor hospital admissions, discharges, and ensure appropriate follow-up
* Maintain accurate patient records and data reporting
* Supportive proactive care initiatives, including frailty management and end of life planning
* Prepare and support vaccinations cohorts with delivery and inputting
* Attend and support monthly MDT Meetings
* Ensure QOF targets are met
* Manage emails and phone calls from both care homes and family
* Triage phone calls and provide safety/escalating information where necessary
* Apply and download 24hr ECG referrals (training will be given)
Person Specification
Experience
* Experience in care coordination, healthcare, or administrative role
* Strong organisational and communication skills
* Ability to work collaboratively within a multidisciplinary team
* Competence in IT systems and patient record management
* Understanding of confidentiality and data protection
* Experience working with care homes or elderly populations
* Knowledge of Primary care networks and NHS Systems
