We are looking for a nurse interested in developing skills in community care. We support newly qualified nurses, return-to-practice nurses, and those transitioning from other settings like acute care. You should be flexible, well-organized, and possess excellent communication skills.

Role responsibilities include providing skilled nursing care based on best practices and clinical evidence, such as chronic disease management, end-of-life care, continence promotion, and wound management. Specific skills required are phlebotomy, medication administration via syringe drivers and IV therapy, PEG management, urinary catheterization, and tracheostomy suctioning.

As a Named Nurse, under supervision, you will be responsible for holistic health assessments, care planning, evaluation, and ensuring continuity of care following referrals from various healthcare settings. You will undertake comprehensive physical, psychological, and social assessments, including carers' needs, and develop person-centered care plans to promote self-care and prevent hospital admissions.
