Senior Practitioner – Preparing for Adulthood Service (PfA)
We are seeking a qualified social worker for the role of Senior Practitioner to join our growing team within the Preparing for Adulthood Service, which provides specialist health and social care services to adults with learning disabilities and complex needs in Hackney.
Responsibilities
* Provide support to practitioners at all career levels, including supervisory skills and deputising for the Team Manager.
* Deliver Care Act (2014) assessments, safeguarding, complex court of protection work, and mental health admissions for young people aged 18‑21.
* Work closely with a multidisciplinary team (psychology, psychiatry, nursing, occupational therapy, physiotherapy, dietetics, speech and language therapy) to ensure quality and robust services.
* Promote an individualized, strength‑based, trauma‑informed practice that maximises independence and wellbeing.
Qualifications
* Qualified social worker, registered with Social Work England.
* Significant post‑qualification experience in statutory adult services, including supervisory/management support to social work practitioners.
* Knowledge of the Care Act, the Mental Capacity Act, safeguarding legislation and other relevant statutes for adults with learning disability.
* Preferable experience working with diagnoses of learning disability, autism or other complex needs.
* Commitment to social work education and integrated working across health and social care.
* Enhanced DBS required.
