Job summary An exciting opportunity to join the award winning Hospital To Home team. The highly performing team provides the social work role within Derbyshire Community Health Services (DCHS) Community First service. The team is based at the Royal Derby Hospital, however Hospital to Home also work with customers across local Community Hospitals, their own homes and within Perth House assessment centre. Your customer base is people over the age of 18 years who require advice, advocacy and support to maximise their independence, safety and well being. The team works closely with the Integrated discharge team and DCHS to discharge customers using the discharge to assess model. Applications are welcomed from candidates with the ability to encourage, support and engage with carers adults both within and recently leaving hospital and people with learning, physical and/or sensory disabilities.
