Are you interested in helping individuals achieve their goals? An opportunity has arisen for a band 3 Rehabilitation Assistant to join Keswick & Solway ICC up to 37.5 hours per week as either a secondment or fixed term contract. The successful candidate will be based with the Solway Community Rehabilitation team working across the beautiful Solway Plain and into the National Park. Based at Wigton Hospital you will see patients within community settings and the well established and innovative Rehabilitation Centre.
We are looking for a motivated and caring individual, preferably with enthusiasm about rehabilitation, problem solving and self‑development. The post involves working as part of a team, collaborating with other teams and servicing and lone working.
The Community Rehabilitation Team is a friendly interdisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission and provide a range of rehabilitation and anticipatory care services.

Main duties of the job
A rehabilitation assistant in a multi‑disciplinary team is expected to work with other health and social care professionals to develop the ethos of multi‑disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard. As a rehabilitation assistant reporting to a registered health professional, the post holder will implement rehabilitation plans for patients in the community and other settings, observe the patient's condition, and provide direct care with a rehabilitation and therapy focus. This may include supporting patients with personal hygiene and activities of daily living.
Flexible working will be required to meet the needs of the patient group.
The ability to work over 7 days is a requirement of the post. Working hours will be on a rota covering a shift pattern between the hours of 8.00 a.m. and 6 p.m.

Disclosure and Barring Service
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows:
* Standard Check: £21.50 + Administration cost of £5.90 = £27.40. Deducted from salary over a 4 month period or a one-off payment.
* Enhanced Check: £49.50 + administration cost of £5.90 = £55.40. Deducted from salary over a 4 month period or a one-off payment.

Qualifications
* QCF level 2 Diploma in Health and Social Care or equivalent qualification in rehabilitation.
* Prepared to undertake QCF level 3 in Health, Social, Diagnostic and Therapeutic Support within 3 years of appointment.
* Evidence of ongoing training and personal development.
* Care Certificate or the willingness and ability to undertake the course.
* QCF level 3 in Health, Social, Diagnostic and Therapeutic Support, or equivalent.
* Additional training in rehabilitation or caring for the older person.
Knowledge and Experience
* Working in a health/social care related post, or worked in a voluntary or informal caring capacity with adults.
* Ability to work with a wide range of people.
* Knowledge and understanding of how an Integrated Care Community functions.
* Basic understanding of the Rehabilitation Assistant's role.
* Knowledge and understanding of the importance of rehabilitation and the promotion of independence.
* An understanding of how to support, with empathy and encouragement, individuals who may be unwell or have reduced functional capabilities.
* Self‑awareness and appreciation of own limitations.
* Knowledge of personal care, function and mobility, nutrition, privacy and dignity.
* Evidence of working within rehabilitation.
* Experience of working within a community setting.
* Experience of lone working.
* Previous experience of working in a multi‑disciplinary team.
* Understanding of reflective practice and its application.
* Knowledge of moving and handling policies and procedures.
