This locum position offers an engaging opportunity to be part of a supportive Adults Hospital Discharge Team focused on seamless transitions from hospital to home or care facilities. Based at Hundens Land in Darlington, you will work full-time within a collaborative environment of Social Workers and community assessment officers, linking closely with an in-house Reablement Service.
What you will do
* Collaborate with a team of Social Workers and community assessment officers to facilitate hospital discharges
* Conduct necessary assessments of individuals’ care and support needs post-discharge
* Travel to hospitals, care homes, and service users’ homes within the Darlington area
* Complete support planning and safeguarding enquiries as needed
* Apply Mental Capacity Act consideration and recording
* Evaluate post-discharge progress to ensure effective reablement and recovery
* Embrace a “Home First” approach promoting independence and well-being
* Report directly to the Team Manager maintaining high standards of practice and service delivery
Requirements
* Degree or equivalent in social work
* Current registration with Social Work England
* Extensive frontline experience in social services within the UK as a qualified Social Worker
