Joint Role - Specialist Frailty & PCN Pharmacist

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

With a particular focus on frailty the Pharmacy team at KCHFT pride themselves with providing a specialist medicines optimisation support and guidance to patients, carers, healthcare and social care staff employed by KCHFT and other organisations through collaborative partnership working. The joint post holder will be expected to support the service requirements as part of their role. Optimising a person's medicines is important to ensure a person is taking and continues to take their medicines as intended. It is a person‑centred approach where shared decision making is an essential part of evidence‑based medicine and needs to be system wide.


Main duties of the job

You will work on location at agreed Community Hospital sites across Kent and at GP Practices within West Kent ABC PCN, Maidstone and remotely as agreed. This is an evolving and forward thinking service and you will support the provision of safe and effective medicines optimisation services to cohorts of patients within these settings, with, where appropriate, opportunity to follow patients across the Primary Care Interface into their normal place of residence. These Medicines Optimisation Strategy goals include:

* Understanding and improving patient, carers and staff experience
* Providing evidence based choice of medicines
* Ensuring medicines use is as safe as possible

The joint post holder will work within their clinical competencies using specialist pharmaceutical knowledge and skills to work with all stakeholders measuring and monitoring medicines optimisation outcomes to improve and achieve the best possible outcome from medicines for every patient.

The outcomes from the goals are:

* Patients take cost effective, evidence based medicines safely
* Patients take only necessary medicines that they agree to take.
* Patients are empowered to manage their medications independently
* Carers, health and social care staff are empowered to support patients to optimise medicines use
* Patients are supported through effective use of their medicines to stay at home and have minimal hospital admissions and re‑admissions.

Job responsibilities

Driving: a full UK driving licence or a full driving licence issued by an EU country (not exchanged from a non‑EU country) is required, and you must be able to travel independently. Person Specification


Qualifications

* Master of Pharmacy (MPharm) degree
* Registration with General Pharmaceutical Council
* CPPE Primary Care Education Pathway
* Postgraduate Clinical Diploma Pharmacy


Experience

* Community, GP practice or hospital pharmacy experience
* Understanding of practical medicines management issues in community based services.
* Experience of delivering medicines optimisation with a patient centred approach
* Experience in education and training of healthcare professionals, social care staff and patient groups
* Experience of working in multidisciplinary teams
* Audit and project management experience


Knowledge

* Clinical Knowledge of medicines use in frailty and chronic diseases
* Knowledge of risk management and assurance issues relating to all aspects of medicines use.
* Knowledge of storage, use and disposal of pharmaceutical products with due regard to local and national policies.
* Familiar with the concepts of evidence-based medicine.
* Practical knowledge and experience in Microsoft Windows/Office products
* Knowledge of the legal and ethical pharmaceutical issues in all areas relating to community health services and GP services


Skills & abilities

* Proven clinical pharmacy skills.
* Ability to work at interfaces between primary, secondary, community and social care including intermediate care.
* Ability to extract and present data from Datix and ePACT2


Personal attributes

* Commitment to mandatory continual professional development (CPD).
* A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non‑EU country) and the ability to travel independently are essential for this role.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
