We are lookingfor dedicated and dynamic Occupational Therapists to join our CommunityOccupational Therapy Team and ourIndependentLivingServices.These roles provide the opportunity to work closely withindividuals in their own homes,supportingthem to regainindependence, improve their quality of life, and prevent unnecessary hospitaladmissions.
Main duties of the job
Key Responsibilities:
* Conduct holistic assessments to identifyindividualneeds and develop person-centredsupportplans.
* Recommend home adaptations, equipment, and assistive technology to promote independence.
* Work collaboratively with multidisciplinary teams, carers, and family members to deliver high-quality interventions.
* Supporthospital discharge planning, preventing readmissions and enabling safe transitions into community settings.
* Promote strengths-based approaches that empower individuals toleadfulfilling lives.
* Provide education and training to staff, service users, and carers on occupational therapy interventions.
What Were Looking For:
* Degree in Occupational Therapy and registration with HCPC.
* Experience working in a community or social care setting (desirable).
* Strong assessment skills with the ability to develop creative solutions to enhance independence.
* Excellent communication andinterpersonalskills for working with individuals, families, and professionals.
* Ability to manage a diverse caseload, balancing urgent needs and long-term interventions.

Experience
* Experience of placements with families and/or vulnerable adults
* Experience of undertaking assessments of individual needs and/or risk under supervision.
* Experience of undertaking assessments with both adult and children.
* Experience of making recommendations for adaptations for adults and children.
* Basic knowledge of a range of medical conditions
Qualifications
* Relevant professional qualification Degree in Occupational Therapy
* Registration as an Occupational Therapist with the HCPC
* N/A
Special Knowledge and Skills
* IT literate regular use of IT systems to support their current job role.
* Ability to work in a pressurised environment.
* Knowledge of the Care Act.
* Ability to work with Technical Officers supporting with plans of work.
