Specialist Occupational Therapist Integrated Community Stroke Service

Eligibility: This role is open only to current NHS employees of the following organisations: Cornwall Foundation Trust, Royal Cornwall Hospitals NHS Trust, and Cornwall and Isles of Scilly Integrated Care Board. Applicants who do not currently work for one of these NHS organisations will not be considered.
As part of the Integrated Community Stroke Service you will participate in the delivery of Occupational Therapy across a defined geographical area, working on a rotational 7‑day basis to provide specialist assessments and care. County‑wide cover may be required on occasion.
Main duties of the job
* Support safe discharges from hospital and provide a high standard rehabilitation service to patients referred to the Integrated Community Stroke Service.
* Provide specialist assessment, effective treatment, and individualised goal‑setting of patients in their own home or place of residence to promote recovery post‑stroke.
* Early identification and management of stroke‑related complications.
* Reduce readmissions due to complications post‑stroke.
* Provide education and support to formal and informal carers.
* Promote the development of self‑management skills following stroke.
* Provide advice and signposting to appropriate services.
Person Specification
Experience
* Experience of working with multi‑disciplinary and multi‑agency teams, with an understanding of team roles and professional roles.
* Experience of evaluating service delivery and monitoring effectiveness and outcomes of intervention, and of undertaking clinical audit and research.
* Experience of working in the NHS in stroke, eldercare, or neurology clinical areas.
Education/Qualifications
* Degree or equivalent qualification in Occupational Therapy.
* Current professional registration with the HCPC.
* Evidence of own professional development and additional specialist training to post‑graduate diploma level.
* Evidence of attending and using specialist knowledge gained from post‑graduate study and courses to support practice.
* Fieldwork educator / clinical placement supervisor / experience as a clinical educator of OT students.
* Membership of the Royal College of Occupational Therapy (RCOT).
* Membership of RCOT Specialist Section – Neurological Practice.
Skills
* Ability to act as an autonomous practitioner to deal with complex clinical issues.
* Completion of stroke competencies or willingness to undertake them.
* The ability to deliver training both within and external to the trust/establishment.
