Surrey Downs Health & Care
We are a multidisciplinary team of Physiotherapists, Occupational Therapists, and rehabilitation assistants within the Primary Care Network, providing integrated care across Surrey Downs. Our rehabilitation services target individuals at risk of hospital admission or recovering post-hospital in their local community.
We offer coordinated care to maximize independence through rapid assessments and support, including moving and handling equipment and liaising with adult social care for additional needs and short-term rehabilitation.
You will lead our Community Therapy services across Surrey Downs, requiring excellent organizational and time management skills, a passion for holistic patient-centered care, and innovative approaches to improve service quality and foster a 'one team' culture. This role is ideal for a proactive individual eager to advance their career within a forward-thinking team.
Main duties of the job
We seek dynamic professionals to drive our Community Therapy services. Key responsibilities include:
1. Co-leading the Occupational Therapy team alongside an existing Band 7 OT and wider team.
2. Working as a vital member of the multidisciplinary team to assess and manage a caseload of clients.
3. Planning, delivering, and evaluating treatments for clients with complex social and health care needs.
4. Contributing to the development and delivery of high-quality, innovative services for the local population.
5. Promoting effective teamwork within Surrey Downs Health and Care, delegating tasks appropriately to benefit patients and service delivery.
6. Opportunities to work across different sectors of the organization through close collaboration and potential rotations.
7. Achieving objectives through clinical practice, education, training, and research.
