Nurse Associate – Complex Care (Home‑based)
Are you a compassionate and experienced nurse associate with a passion for complex care? Step into a pivotal role where your expertise will transform lives. We are looking for a nurse associate to join our clinical team, helping us deliver safe, effective, and person‑centred care for children and young people with complex health needs.
Location
Home‑based with travel across Leeds.
Hours
Full‑time, Monday–Friday 9am – 5:30pm.
What You’ll Be Doing
* Supporting the mobilisation of new care packages, including care planning and risk assessments.
* Training and mentoring care staff, ensuring competence in specialist clinical interventions and complex care.
* Developing and reviewing care plans and risk assessments.
* Monitoring and auditing care delivery to the highest standards.
* Building strong relationships with the people we support, their families, and multidisciplinary teams.
* Managing a caseload that includes a mix of adult and paediatric packages.
What We’re Looking For
* Nursing associate with a current NMC registration and experience in complex care or learning disabilities (adult or paediatric).
* Training / competency sign‑off experience preferred.
* Confident in writing and reviewing clinical documentation.
* Excellent communication skills with people we support, families, and teams.
* Strong organisational skills to manage your diary and priorities effectively.
* Knowledge of CQC regulations and commitment to the NMC code of conduct.
* Computer literacy (Word, Excel, Outlook).
