Locum Social Worker - Leaving Care Team
Are you a passionate and committed Qualified Social Worker with experience supporting care-experienced young people?
We are currently recruiting for an experienced Leaving Care Social Worker to join a supportive and forward-thinking Leaving Care Team in Halton. This is a rewarding opportunity to work with young people as they transition into independence, helping them achieve positive and sustainable outcomes.

Responsibilities:
* Manage a caseload of care-experienced young people
* Develop and review Pathway Plans in line with statutory requirements
* Support young people in areas such as accommodation, education, training, and employment
* Promote independence and life skills development
* Work collaboratively with partner agencies and professionals
* Maintain accurate and timely case recordings and reports
* Advocate for young people, ensuring their voice remains central
To be considered for the role, ideally you will have:
* Diploma / Degree in Social Work
* Social Work England registration
* Experience working with Leaving Care / Care-Experienced Young People
* Strong understanding of relevant legislation and statutory duties
* Ability to build effective relationships with young people
* Good organisational and communication skills
* Ability to work independently and as part of a team
