Job summary
An excellent opportunity has arisen for an enthusiastic, stroke nurse to join our community stroke team to work 37.5hrs over 5 days on a rota system.
The ability to use your initiative, work proactively, demonstrate excellent communication and influencing skills, manage and prioritise your own workload and respond flexibly to changing demands are essential requirements of the post.
You will predominantly work from base/with stroke survivors in their own homes, but there will also be an opportunity to work from home for which you will be provided the necessary equipment to do so.
The role will provide you with variety and no two days are the same.
As a trust we offer a robust induction period and continue to promote opportunities for development through 1:1 supervision session, peer working and in-service training
The trust offers regular training opportunities via Learning and Development to promote your clinical and non-clinical skills
If you would like to discuss informally if this is the right role for you, please get in touch with the recruiting manger which can be found on the top right-hand side of the advert.
This is full time post, but we would consider applicants looking for part time.
This post requires working rotational shifts.
Main duties of the job
Day to day, you will be undertaking comprehensive assessments of mild-moderate stroke patients including those with multiple pathologies/co-morbidities. You will be supported by more senior Stroke Nurses to be able to assess risk factors for stroke, working with stroke survivors and their families in giving secondary prevention education whilst promoting patient advocacy and empowerment. You will also be actively supporting stroke survivors with any altered mood issues to enable stroke survivors to lead purposeful and full-filling lives.
You will attend the weekly MDT in which the team will discuss the progress of patients and identify goals that are achieved and ongoing. You will also need to liaise with other health care professionals within the Trust/Acute Trust and third party agencies such as The Stroke Association.
We pride ourselves on being truly interdisciplinary, so you will also be involved with supporting patients to undertake their morning routine, support with medications and/or facilitate their meal preparation to promote independence. These sessions are part of a rehabilitation focused approach of which we can evidence great results in patients regaining independence. You will be given the appropriate training and support to do this.
