Overview
Aberdeen City Out of Hours Nursing Band 6 Senior Community Nurse. There is an opportunity for Band 6 Senior Community Nurses to join our Aberdeen City HSCP Evening and Night Community Nursing Unscheduled Care Team.
Role and duties
The Senior Community Nurse works with and supports Band 5 and Band 3 colleagues delivering a person centred clinical nursing service, which prioritises end of life care and symptom control for patients in their own homes and plays a vital role in supporting admission avoidance. The role includes triage and management of unscheduled nursing referrals from GMED, NHS 24, Hospital at Home and other professionals, and provides support for the District Nurse with team management responsibilities.
Qualifications and experience
Candidates must possess a Graduate Diploma in Integrated Community Nursing, or a relevant Post Graduate Certificate and evidence current clinical experience in a Community Nursing role at Band 6, which includes triaging of referrals, workload planning, palliative and end of life care.
Working hours and location
Hours: 1800-0000, 2 or 3 shifts within an 8 day rota.
Location: Aberdeen Community Health and Care Village, White Zone, Frederick Street, Aberdeen AB24 5HY.
Requirements
Car owner or access to Car/driver and current UK driving license is required. Car must be LEZ compliant.
