Job Title: Occupational Therapist - Lifelong Services Reviewing
Location: Horsham
Pay Rate: 38 per hour
Liquid Personnel is seeking an experienced Occupational Therapist to join our client's Adult Lifelong Services reviewing team. As part of our Countywide Review Team, you will work with adults (18+) who have physical disabilities, learning disabilities, acquired brain injuries, and chronic health conditions. Your role will include conducting strengths-based reviews, performing Mental Capacity Act assessments, and contributing to Continuing Health Care assessments.
Key Responsibilities:
* Conduct strengths-based reviews and develop support plans.
* Perform Mental Capacity Act assessments.
* Contribute to Multi-Disciplinary Continuing Health Care assessments.
* Collaborate with the reviewing service team.
* Carry out risk assessments and provide recommendations for assistive equipment and home adaptations.
Qualifications:
* Qualified Occupational Therapist with HCPC registration.
* Minimum 2 years of experience.
* Enhanced DBS check.
* Ability to travel as required.
Skills and Experience:
* Strong communication and organizational skills.
* Proficient in case management and report writing.
* Ability to work under pressure and manage multiple referrals.
* Experience with community settings and social care services is desirable.
