Band 6 Care home liaison nurse: Community adult nursing

Department: Inverclyde HSCP - Community adult nursing

Role of the department:

Caring is fundamental to nursing services and to all nursing functions. The core values of nursing must underpin the practice of every nurse and drive care that promotes positive and equitable engagement with patients, families and carers as the central focus of practice. The community nursing service operates 24 hours per day providing care to patients within their own homes. Community nursing services offers a wide range of skilled clinical interventions which encompass a broad spectrum of technical expertise, health education and health promotion to maximise the people of our community’s independence and support carers. The community nursing service works collaboratively and flexibly with all disciplines to support the people who live in Inverclyde.

The job the applicant will be doing:

1. An exciting opportunity has arisen for an enthusiastic, motivated registered nurse to work within community nursing in the Care Home Liaison Nursing service within Inverclyde HSCP.
2. The post holder will use a collaborative and co-ordinated approach to provide enhanced support and clinical advice to care home staff to enable high quality standards of care to residents with a focus on prevention of avoidable hospital admissions, enhancing quality of life for residents and supporting staff in our care homes with person centred care.
3. The care home liaison nurse will work in partnership with other relevant staff members of the multi-disciplinary team and care home to ensure safe and effective care using a quality improvement approach to improving outcomes for residents.
4. The ideal candidate will hold an adult nursing registration with considerable relevant experience and transferable skills and will have or be willing to undertake further professional development related to the role in line with:

7. Working in partnership with Social and care home staff the post holder will support General Practitioners and other colleagues in the ongoing provision of enhanced services for residents within care homes.
8. The post holder will be expected to provide an accessible, appropriate, high quality, culturally sensitive, safe, person centred and effective service in the community, by working in partnership with people and families, care providers and other agencies.
9. The ideal candidate will have a minimum 4 years post registration experience, preferable but not essential within the community setting. The post holder will be educated to degree level and must possess / be willing to work towards the qualifications of independent prescribing and advanced clinical assessment.
10. Driving license holder/driving is essential to this post.

Skills / Qualifications:

A 1st level registered nurse with significant post registration experience is essential. Community experience is essential. An interest in Palliative Care and Management of Long Term Conditions / Anticipatory Care alongside associated technical skill & ability is considered advantageous. E.g. enteral feeding T34 Syringe Pumps. As part of NHS Greater Glasgow & Clyde’s pre-employment checking for all appointed candidates, evidence of current NMC registration will be required prior to start date. You should therefore note that candidates who are unable to provide confirmation of current NMC registration, in line with projected start date for this post, may not be shortlisted for interview. This post requires PVG Scheme Membership / Disclosure Scotland Check. Full driving licence essential.
