The Short-term assessment and reablement team in the High Peak is looking to recruit a qualified and experienced social worker to join our friendly and supportive team.
This is a 22-hour and 12-minute part-time role covering all areas of the High Peak, including Glossop, the centre of High Peak, and the south, including Buxton.
The role involves handling all new adult care assessments to determine support needs for individuals aged 18 and above, including safeguarding and capacity assessments in line with the Mental Capacity Act 2005.
Responsibilities include engaging in short-term home care support, general social work, conducting mental capacity assessments, and safeguarding inquiries.
The successful candidate will demonstrate the necessary skills, knowledge, and experience to provide a safe, accountable, cost-effective, and efficient service, focusing on person-centred and strengths-based approaches to help clients maintain and maximize independence.
Key responsibilities:
1. Carrying out Care Act 2014 assessments and reviews for individuals with complex needs.
2. Arranging services to meet eligible needs.
3. Working collaboratively with clients and families to support independence.
