Key Working Relationships

* DPT MO team and medicines optimisation link practitioners
* DPT multi-disciplinary community teams (CMHT and Rehabilitation) including Consultant psychiatrists, other DPT medical and non-medical prescribers, nursing staff, occupational therapists, social workers and psychologists
* PCN GPs, pharmacy staff and other primary care staff
* Other specialist Mental Health and Learning Disability pharmacists through specialist interest groups/clinical supervision network
Responsibilities
* Provide a highly specialist, clinical pharmacy service to mental health and rehabilitation services, including community mental health/learning disability and primary care.
* Identify and minimise risks associated with the use of medicines; contribute to medication care plans; work as an integral member of the multi-professional team.
* Review medication charts and prescription records to ensure prescriptions are appropriate, legal, legible, safe and effective; identify drug interactions and adverse effects and address issues as appropriate.
* Advise on monitoring of medication regarding efficacy, safety and side effects; interpret health monitoring and blood test results.
* Undertake medication reviews and drug history reports as required; document interventions in the electronic patient record and communicate according to policy.
* Educate people who use services and their carers about medication; develop schemes promoting medicines adherence.
* Promote seamless care by liaising with GPs, nurses, other specialties and community pharmacists; ensure clear treatment plans, safe prescribing, continued prescribing/supply arrangements, and medicines reconciliation.
* Provide a medicines information service for community staff; support PCNs with safe practice (e.g., SOPs) for managing mental health medicines (e.g., depots, clozapine).
* Practice as an independent pharmacist prescriber when qualified; provide timely access to treatment in line with policy.
Policy, Service Development and Governance
* Recommend and contribute to the development, implementation and review of medicines-related policies, clinical protocols, SOPs, prescribing guidelines and safety briefings.
* Ensure adherence to Medicines Policy standards and report deviations as required; review medicines incidents and contribute to RCA where necessary.
* Attend governance meetings to address medicine-related risk areas and implement changes to practice.
Finance, Equipment and Other Resources
* Ensure medicines are stored and used in accordance with policy; assist with prescribing reports detailing clinical and financial information.
* Advise on measures to improve cost-effectiveness and participate in cost-improvement initiatives; support budget interpretation and highlight variances and remedial actions.
* Promote cost-effective prescribing at MDTs consistent with standards.
Human Resources
* Managed by the Deputy Chief Pharmacist Clinical Services; may deputise as needed and attend locality meetings.
* Provide line management for technicians and pharmacists within the workstream; provide clinical supervision and work autonomously with support as required.
* Develop and deliver medicines-related education and training; supervise technicians and trainees from related settings.
Information Resources and Administrative Duties
* Contribute to a DPT central medicines helpline; ensure records comply with information procedures and confidentiality.
* Ensure entries in clinical notes follow standards; reference evidence-based medicine and guidelines (NICE, Cochrane) and advise on complex clinical situations.
* Ensure minutes are accurate and action logs followed up.
Research and Development
* Regularly undertake research or development activities; ensure clinical practice is evidence-based.
* Keep up to date with research; conduct complex audits; analyze prescribing data with the data analyst to identify improvements.
Analytical and Judgemental Skills
* Review prescriptions for appropriateness; advise on treatment choices, dosages and legality of prescriptions (e.g., Mental Health Act considerations).
* Collaborate with community teams and PCN practices to review treatment plans according to guidelines; ensure complex regimens are evidence-based and challenge prescribers when necessary.
* Advise on next steps for treatment-resistant cases and communicate with patients in accessible language; work with data analysts to identify issues.
