**Job Summary**
The post holder will be a member of the South Durham and Darlington - Early Intervention in Psychosis Team, managing a caseload of service users aged 14 to 65 experiencing a First Episode of Psychosis.
**Responsibilities**
- Conduct initial and comprehensive assessments of health and social care needs in collaboration with colleagues, providing feedback to the Multi-Disciplinary Team.
- Responsible for care coordination for a specific group of service users, supporting recovery and social inclusion.
- Participate in MDT activities, including duty and clinic rota as appropriate.
- Develop and maintain effective communication strategies within and between mental health services and GP teams to enhance care pathways.
- Work within TEWV policies, procedures, and legal frameworks, upholding professional values and ethos.
- Practice within the Code of Professional Conduct, safeguarding protocols, and relevant Acts (Mental Health and Capacity Acts).

- Salary: £29,970 to £36,483 per annum

**Qualifications & Experience**
- Registered with NMC (RNMH, RNLD, or RGN as appropriate)
- Experience in mental health or learning disabilities, working collaboratively, and in multidisciplinary teams.
- Knowledge of relevant legislation, safeguarding, clinical risk assessment, and medicines management.
