We’re looking for a passionate and proactive person to join our Health and Local Crisis Response Team as a Service Coordinator. Imagine being at the heart of our team, connecting with hospital staff, medical experts, and community members, including GP surgeries. As the go-to person for patient referrals, you’ll play a crucial role in ensuring a smooth and efficient process. You’ll evaluate the impact of our services and look to fine-tune and improve where needed to drive positive change.
Overall purpose of the role
The Coordinator will be responsible for making settling-in calls to patients who have recently been discharged from hospital to check on their patient experience and interactions with health and care services. Insights captured through these calls will be fed back to relevant system partners to support continuous improvement of health and care services. While making the settling-in calls, you will ensure individuals are safe and well at home and that their needs are met. If needs are identified, you will refer and signpost to other statutory or non-statutory services to ensure the right level of care.
You will liaise with hospital staff and other medical professionals to receive patient referrals. Thorough records are required to evaluate and analyse outcomes of the services. You will also assign incoming referrals to a Service Assistant (or equivalent), monitor service delivery, manage the service budget, ensure service health and safety, evaluate progress, oversee record-keeping, recruit and manage volunteers, attend hospital meetings, and ensure the team works together seamlessly.
What will a day in the life look like?
* Responsible for building strong positive relationships and liaising with hospital staff and other medical professionals in the community, including GP surgeries, to receive patient referrals.
* Managing a team of staff and volunteers.
* Evaluating and analysing the outcomes of the services, assigning incoming referrals to support workers.
* Monitoring the service delivery, ensuring service health and safety, and evaluating progress.
* Overseeing record-keeping and recruitment.
Requirements
* IT literacy.
* Understanding of how to improve service quality for the benefit of users.
* Full UK driving licence holder and access to vehicle.
* Willingness to work flexible hours.
* Ability to deal with queries in a diplomatic, professional and confidential manner.
