Staff Nurse (Adult Community Services - Day Hospice & Hospice at Home)

We have an exciting opportunity for a part time staff nurse tojoin our friendly Hospice at Home and Day Hospice team at the ShakespeareHospice.

As a staff nurse, you will be a core member of themultidisciplinary palliative care team. The role will involve working acrossour traditional Hospice at Home and Day Hospice services covering SouthWarwickshire. Working in close collaboration with our other Hospice andcommunity teams, the post holder will promote holistic, person centred care forpatients, their families and those who require specialist palliative support.

We providecare for patients with life limiting illnesses, also offering carer andbereavement support. Our staff nurses are key members of the team and areexpected to work closely with the Team Leaders to maintain excellent standardsand develop future service provision. The post holder will uphold our hospicevalues: to be compassionate, accessible, respectful and effective.


Main duties of the job

To actas a role model and provide exceptional nursing skill and care across TheShakespeare Hospice Adult Community Services traditionally Hospice at Home andDay Hospice

Toprovide physical, psychological, social and spiritual care for patients andcarers.

To holisticallyassess, plan, implement and evaluate patient care, continuously reviewingpatient-led goals and outcomes through effective care planning.

Toundertake specialist patient centred assessments, using an evidence-basedpractice approach, to support patients during end of life care, or whereappropriate in maintaining and/or maximising their independence and quality oflife.

Toconfidently identify and respond to changes in the patients condition andsymptoms, adjusting care and nursing interventions accordingly and with theknowledge of the Team Leaders.

To respondappropriately in managing patient crisis/deterioration and to ensure that aneffective plan of care is in place.

Toensure that all necessary and recommended infection control precautions aretaken at all times.

Toconfidently liaise with other relevant professionals and statutory/voluntaryorganisations, as necessary, to facilitate appropriate patient care.

Toparticipate and promote advanced care planning discussions with patients andcarers, considering and respecting their priorities of care and individualpreferences at all times.

As a member of The Shakespeare Hospice team you will possess excellent communication skills and be a dynamic team player who isenthusiastic and passionate about palliative care. You should be driven and keen to continuouslyimprove practice and advocate on behalf of patients. Eager to both learn anddevelop whilst supporting other team members in their day-to-day practice. You will need to be flexible and comfortablewith working daytime shifts (8.30am to 16.30pm; 14.00pm to 22.00pm), over a7-day rolling week. This post does notrequire any overnight shift work but does involve some evenings.


Job responsibilities

Summary ofRole

As a staff nurse working within TheShakespeare Hospice Adult Community Services, you will be a core member of the multidisciplinarypalliative care team. The role will involve working across our traditionalHospice at Home and Day Hospice services covering South Warwickshire. Working in close collaboration with our otherHospice and community teams, the post holder will promote holistic, personcentred care for patients, their families and those who require specialistpalliative support.

We provide care for patients with lifelimiting illnesses, also offering carer and bereavement support. Our staff nurses are key members of the teamand are expected to work closely with the Team Leaders to maintain excellentstandards and develop future service provision.The post holder will uphold our hospice values: to be responsive,compassionate, supportive, collaborative and innovative.

Main Dutiesand Responsibilities

Clinical

Tooffer holistic support to carers and bereaved carers, identifying andresponding to their individual and fluctuating psychosocial needs.

Toutilise national standards and NICE guidelines to underpin evidence-basedpractice.

Toutilise a range of effective communication and listening skills to facilitate complexand sensitive conversations.

Tocomplete contemporaneous and accurate clinical records.

To beactively involved in caseload management and patient flow from point ofreferral to discharge.

Toprioritise workload accordingly and to work flexibly to meet the changing needsof patients and the organisation.

To beable to work both autonomously and within close partnership with the Hospiceteam.

Toactively participate in relevant multi-disciplinary team meetings, teamhandovers and case conferences to ensure optimal patient care.

Toassist in the delivery of health promotion and educational programmes topatients, families and carers.

To be asource of expertise and an advocate for hospice palliative care.

Managerial

Tosupervise and support other members of the team, including both staff andvolunteers, acting as a role model demonstrating our Hospice values at alltimes.

Tosupport junior staff members, as delegated to you by the Team Leaders, with1to1s, appraisals and objective setting.

Todeputise for the Team Leaders, with support from other senior staff asrequired.

Todemonstrate proficient IT skills in order to maintain and develop data collectionand reporting.

Tosupport the Team Leaders in local audit and evaluation, assisting with anyimprovement plans that might be identified.

To takea proactive approach towards service development, using initiative andcreativity to implement new ideas and ways of working, where mutually agreedwith the Team Leaders to be of benefit to patients, staff and the organisation.

Organisational

Todemonstrate on a day to day basis the organisational values of the Hospice,acting as a role model to others.

Tosupport the organisation to ensure that all clinical staff and clinicalvolunteers are appropriately equipped and trained to fulfil their duties.

Toundertake administrative tasks for the purpose of your role and utilisevolunteer support as appropriate.

To workin accordance with all Hospice policies and procedures.

To workin accordance with the Nursing and Midwifery Council Professional Code ofConduct, maintaining professional requirements to comply with revalidationrequirements.

Toparticipate in the Hospice appraisal process, responsible for complying withmutually agreed personal objectives.

Toactively participate in clinical supervision, both in group scenarios and inone to one provision.

Toraise any areas of risk or concern in accordance with hospice policy, includingincident reporting.

To be involvedwith the wider organisational aspects of the hospice, such as promotion of thecharity and its services. This mayinclude representing the hospice at certain key events, where clinical supportis required.

Educational

Toadhere to all statutory and mandatory training requirements as designated bythe organisation.

To beresponsible for your own Continuous Professional Development (CPD).

Toattend and participate in learning and development opportunities as deemedmutually beneficial for both yourself and the organisation.

To keepabreast of current policy and key developments in palliative care, both locallyand nationally.

To beprepared to teach and share knowledge with the wider teams, within own scope ofconfidence and competence, and with support from other team members.

Toactively engage with the assessment and supervision of nursing students,medical students and to take a key role in supporting work experiencestudents.

To be involvedin the induction programmes for new staff members and volunteers.

Other

Person Specification


Qualifications

* Degree or Diploma in Adult Nursing (RGN)
* Evidence of current NMC registration
* Evidence of ongoing CPD
* Evidence of further relevant post-qualification training, such as oncology, palliative care, bereavement support
* Mentoring / Assessing qualification or equivalent


Experience

* Previous experience in palliative/end of life care.
* Ability to organise work and develop programmes of care.
* Experience of working in a variety of settings, including community settings and with a variety of patient groups.
* Ability to reflect and adapt practice to suit individuals needs.
* Experience of multi-disciplinary team working.
* To advocate professionally on behalf of patients
* Ability to work autonomously and as part of a team.
* Ability to supervise other members of staff and volunteers
* Ability to identify risks and risk management

* Knowledge of range of palliative conditions, including oncology, neurology, respiratory and cardiac conditions

£29,970 to £36,483 a yearpro rata, dependent upon experience
