We are seeking a Nurse Practitioner to join a multidisciplinary team supporting clients affected by Hepatitis C.
Location: Scarborough, ON
Hours: 35 hours per week
Compensation: $51.29–$72.65/hour
Employment Type: Permanent, Full-Time
About the Role
We are seeking a Nurse Practitioner with a passion for community health and harm reduction to join a multidisciplinary team supporting clients affected by Hepatitis C. This role focuses on education, diagnosis, treatment, and ongoing support—improving outcomes and reducing transmission through compassionate, evidence‑based care.
Key Responsibilities
* Deliver client‑centered, trauma‑informed care within full NP scope of practice.
* Provide diagnosis, treatment, and ongoing follow‑up for clients living with or at risk of HCV.
* Support patients in learning to administer treatments and manage side effects.
* Conduct education and outreach sessions on prevention and wellness.
* Maintain clinical documentation and participate in chart reviews and quality audits.
* Collaborate with interprofessional care teams and community partners.
* Provide telephone support and on‑call coverage as required.
Qualifications
* Master’s degree with Primary Health Care Nurse Practitioner certification.
* Active registration with the College of Nurses of Ontario (Extended Class).
* 3–5 years’ experience as an NP in community, primary care, or harm reduction settings.
* Strong understanding of health promotion and public health principles.
* Member of NPAO with professional liability coverage.
* Valid driver’s license and vehicle access required.
