Due to expansion of the practice we are recruiting within our pharmacy team. We have a full-time (or can be part-time) position available to join our innovative and progressive practice.

We are a multisite GP Practice with a list size of circa 45,000 patients, and operate a triage model of care supported by a multi-disciplinary team including GPs, ANPs, pharmacists and physiotherapists to name a few. We are looking for someone who would like the opportunity to have an impact on the future development of General Practice.

We are a dynamic practice responding in a creative way to the changing demands of primary care by:

1. Utilising concepts of demand and capacity management
2. Clinical & social integration in hub-based working
3. Providing quick patient access with no appointment backlog

Main areas in this role would encompass but not limited to:

4. Identify and review patients with medication related issues (including patients with polypharmacy)
5. Triage and where appropriate, manage acute prescription requests within the practice
6. Ensure a safe, robust and efficient repeat prescription system within the practice and contribute to the regular medication review process
7. Conduct medications reconciliation for patients discharged from hospital and newly registered patients.
8. Deal with outpatient clinical requests
9. Deal with queries from community pharmacies, hospital pharmacists, care homes, patients and carers
10. Supervise other members of the pharmacy team e.g. Pharmacy Technicians and Pharmacy Clerk
11. Carry out quality improvement activities/audits within the practice
12. Be involved in care home medication management. This includes carrying out medication reviews, helping with processing of monthly medication requests and attending multidisciplinary review meetings
13. Chronic Disease Management

Independent prescribing would be preferable, however mentorship will be available if wishing to complete the independent prescribing course. Mentorship will also be available to develop skills in the art of telephone consultation and triage, clinical decision making and risk assessment.
