About us
Situated in the semi‑rural village of Duffield (5 miles north of Derby), we serve a population of just over 11,000 patients. We are in a modern, purpose‑built premises with free parking and disabled access. We have an attached (independent) pharmacy unit on‑site. We also operate from a small branch surgery in the neighbouring village of Little Eaton. We are within easy commute of Derby and Nottingham and are situated close to the beautiful Derbyshire Peak District.
Job Summary
The Practice Nurse will work 21‑28 hours per week. Hours are on Tuesday‑Friday, with Friday non‑negotiable. The role involves providing nursing care to patients, including treatment, preventative care, screening, and patient education, under established protocols.
Job Responsibilities
* Routine patient care and nursing tasks
o Basic wound care / ulcer care
o Removal of sutures
o New patient medicals
o Urinalysis
o ECGs
o Peak flow readings
o Venepuncture
o Blood pressure measurements
o Ear syringing
o Routine immunisations
o Health checks
o Cytology
o Assisting GPs with minor surgery and coil fittings
o Requesting basic pathology tests (e.g., urine culture, swabs)
o Follow agreed clinical protocols with referral to senior nurses or GPs as appropriate
o Take part in the annual flu immunisation programme
o Support the work of other nursing staff during slack periods
o Provide cover for other clinics within your hours or as overtime if required by the Practice Manager
o Keep skills up‑to‑date for all nursing duties you might be required to undertake (e.g., smear‑taking, wound care)
o Carry out over 75 checks as required
o Provide emergency nursing services (e.g., ECGs, bloods)
* Chronic disease management
o Understand and manage coronary heart disease, cardiovascular disease, hypertension, diabetes, respiratory diseases, chronic kidney disease, peripheral arterial disease
o Maintain practice templates for chronic disease management and update at every opportunity
o Provide written and verbal information about diagnosis and treatment
o Offer lifestyle risk reduction advice
o Work within agreed protocols for risk‑factor management
o Undertake annual reviews of CDM patients
* Diabetes management
o Maintain up‑to‑date register of diabetic patients and correct coding
o Monitor health against key indicators per practice protocol
o Advise and support patients for self‑management and control
o Arrange recall for annual reviews and monitor attendance
o Liaise with other health professionals for delivery of diabetic care
o Contribute to development of practice diabetes protocols and audit
o Ensure adequacy of equipment and supplies for diabetic care
* Warfarin management
o Initiate and manage warfarin therapy using established protocols
o Adjust dosing based on INR and clinical context
o Recognise clinical emergencies and coordinate with GP
o Advise on coordination of patient care and referral when needed
o Use computerised dosing software to an advanced level
o Apply national, regional, and practice guidelines for anticoagulation
* Sexual health
o Advise patients on cervical cytology and mammography screening programmes
o Perform cervical cytology sampling under supervision until competent
o Audit smears to maintain adequacy rate <12.7%
o Promote breast self‑awareness and provide literature
o Advise on sexually transmitted diseases and referrals
o Take swabs for microscopy as directed
o Give advice on peri‑menopausal and post‑menopausal symptoms and referrals
* Family planning
o Provide advice on contraception methods and referrals
o Perform interval health checks for hormonal contraception users
o Support emergency contraception attendance and future contraceptive advice
* Vaccinations & immunisations
o Provide adult and child immunisation service
o Help maintain call and recall of patients (with admin and reception teams)
o Develop and follow practice protocols (cold chain, anaphylaxis)
o Ensure cold chain protocol and anaphylaxis training every 3 years
o Administer injections under prescription or patient group direction
o Maintain accurate records per protocol
o Identify at‑risk groups for influenza and pneumococcal and promote uptake
o Provide travel vaccinations and advice on travel health risks
o Signpost patients to private travel vaccination services where NHS vaccines not available
* Respiratory disease management (Asthma & COPD)
o Maintain up‑to‑date knowledge of asthma and COPD management
o Undertake annual reviews
o Perform peak flow readings, FENO and spirometry (where trained)
o Assess inhaler technique and provide education
o Develop personalised asthma and COPD self‑management plans
o Identify patients requiring escalation or referral
o Provide smoking cessation advice
o Maintain disease registers and accurate coding
o Participate in recall systems for respiratory patients
* Other tasks
o Maintain and clean equipment used by staff
o Maintain GP and nurse rooms, stocking and rotating items
o Chaperone and assist patients being examined by another clinician
o Undertake housekeeping duties (tidy and clean nurse and treatment rooms)
o Order vaccinations to maintain stock levels
o Participate in administrative systems in the practice
o Maintain accurate records at all times
o Advise senior nurses of potential problems or errors within assigned tasks
o Attend and participate in practice meetings when required
o Occasionally undertake home visits

Qualifications
* Registered General Nurse
* Recognised Training/qualification in Cervical Cytology screening
* Recognised Training/qualification in Anticoagulation management
* Recognised Training/qualification in Chronic Disease Management (including Asthma and COPD)
* Recognised qualification in diabetes management
* Recognised Training/qualification Women's Health (include Contraception and HRT)
* Recognised Training/qualification in administering adult and childhood vaccination
Experience
* Experience of computers and clinical systems
* Competent in basic Practice Nurse Duties (including ECGs, wound care, flu vaccinations)
* Able to work without direct supervision and determine own workload priorities.
* Understands the need to maintain data security and confidentiality of information.
* Practical knowledge of SystmOne (clinical system)
* Experience of working in a General Practice
