We’re looking for a Private Client Care Coordinator to join the team and coordinate all aspects of our client’s care package under the direction of the Clinical Lead. The client has a complex cardiac, gastrointestinal, and respiratory history, requiring a well-coordinated, highly reliable nursing service. Care is delivered within a private residence, supported by a small, established nursing team. The household also includes a small, friendly dog, and applicants should be comfortable working in a home environment. We provide holisitic support from professionals our clients can trust, whether it is temporary post-operative support, or a loved one requiring long-term palliative care in their own home, our team works with clients to create a care service tailored to their needs. About the Role In this role, you will: Coordinate the day-to-day running of the client’s package, ensuring all key operational elements are organised, communicated, and followed through and act as a key point of contact for the client, their family and the care team. Support the Clinical Lead by coordinating actions arising from reviews, meetings, incidents, concerns, client changes, and package developments. Support the coordination and maintenance of package documentation, trackers, action logs, and reports. Support oversight of non-clinical package logistics, including coordination around supplies, equipment, household arrangements, or other practical requirements relevant to the smooth running of the package. Experience coordinating complex care packages, client services, or high-level support arrangements. Experience working with clients in community-based settings.
