Work Schedule:
100% FTE, days. Hours are 8:00 am to 5:00 pm, Monday through Friday. You may be eligible for a sign-on bonus of up to $3,000 (pro-rated based on FTE). This is a hybrid role; when in person, you will work at the DeForest-Windsor Clinic in Windsor, WI.

This position is part of the Collaborative Care workstream in Primary Care. As a member of the team, you will collaborate with primary care staff and team psychiatrists to deliver brief interventions for patients.
We are seeking a Behavioral Health Therapist to:
* Collaborate with other medical professionals in a fast-paced environment.
* Provide brief, evidence-based interventions.
* Conduct short, intensive follow-ups.
* Connect patients to resources.
* Use outcome-based models to track patient improvement.

Qualifications
* Master's Degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related behavioral health field (Required)
Work Experience
* 2 years of experience providing evidence-based treatment for various behavioral health diagnoses for children, adolescents, or adults (Required)
* 5 years of experience preferred
Licenses & Certifications
* Licensed Clinical Social Worker (Required upon hire) or
* Licensed Professional Counselor (Required upon hire) or
* Licensed Marriage and Family Therapist (Required upon hire) and
* Basic Life Support/CPR (Required within 6 months)
* Substance Abuse Counselor (Preferred) or
* Clinical Substance Abuse Counselor (Preferred)
