We are looking to recruit a dynamic and experienced Senior Occupational Therapist to join our supportive, forward-thinking Frailty Team. You will demonstrate excellent communication and prioritisation skills, alongside a flexible, person-centred approach to care.

We are open to Full Time, Part Time and BANK applicants for this role.

This is an exciting opportunity for an individual seeking a new challenge, wishing to broaden their clinical experience. The role offers the chance to develop a wide range of skills across multiple pathways, supporting professional development and career progression within the service.

You will work primarily within the Frailty Team, undertaking preventative assessments to reduce the progression of frailty and avoid unnecessary hospital admissions. You will deliver timely, short-term interventions aimed at maintaining patient safety and independence at home.

There are also opportunities to contribute to other areas of the service, including supporting safe and timely hospital discharges.

You will be part of an expanding Frailty Hub, comprising anticipatory care, crisis response, and discharge pathways. The team works collaboratively with primary and secondary care services, social care, the voluntary, community and social enterprise sector, and Yorkshire Ambulance Service, ensuring an integrated approach to patient care.

Main duties of the job

* Carry out thorough assessments, including complex cases, to understand patients needs and create effective treatment plans.
* Plan and deliver personalised, goal-focused treatment using evidence-based practice.
* Work closely with the wider multidisciplinary team, including social care and voluntary services.
* Provide advice and training to support lifestyle changes, helping patients improve daily living, work, education, and leisure skills.
* Manage and prioritise your caseload, adapting plans as needed and delegating tasks appropriately.
* Motivate patients and involve families/carers to support engagement in treatment.
* Communicate clearly using a range of methods, including with patients who have communication difficulties.
* Follow professional standards (RCOT and HCPC) and take responsibility for safe, effective practice.
* Support the team with complex cases and act as a point of contact for clinical queries.
* Monitor and evaluate your work using audits, outcome measures, and evidence-based practice.
* Provide supervision, support, and training to colleagues and students.
* Train patients, carers, and staff in the safe use of equipment.
* Keep up to date with best practice, especially in frailty care.
* Take part in and deliver training and development activities.
* Support service improvement through audits, research, and quality initiatives.
* Step in for senior staff or team leads when required.
* Recognise when to escalate issues beyond your role.

Person Specification


Qualifications

* BSc/MSc in Occupational Therapy (HCPC) registered
* Membership of Royal College of Occupational Therapists (RCOT) Postgraduate training in frailty management, dementia care, falls prevention, or palliative care Training in mentorship, or clinical supervision
* Teamwork: Works effectively as part of a multidisciplinary team, demonstrating flexibility and willingness to support colleagues across urgent care, surgery, and wider clinical teams. Autonomy: Makes sound clinical decisions independently, drawing on professional knowledge and judgement while recognising professional boundaries. Time & Workload Management: Manages unpredictable and fluctuating caseloads effectively, adapting to changing priorities and using appropriate communication methods. Skills and Abilities

* Communication Skills: Able to calmly, professionally, and efficiently identify and respond to patients and internal stakeholders needs. IT Skills: Competent in Microsoft Office applications and confident in learning new clinical systems. Able to accurately input, interrogate, and manage data with strong attention to detail. £38,880.72 to £41,033.12 a year Pro Rata if part time working, salary is dependant on experience
