Nutrition Case Manager
Fixed Term Contract - up to 10 months Maternity cover
Location: Tewkesbury, Gloucestershire
Salary: £27-35k depending on experience
We are looking for someone who is passionate about Nutrition to join our Corporate Health division on a fixed term contract basis as maternity cover. The Nutrition Case Managers support delivery of Nutrition services across all business service channels through consultations, training and coaching as part of our integrated health and wellbeing model. This is a varied role which would suit an organised and problem-solving clinician who understands how good nutrition can contribute to good overall health and wellbeing inside and outside the workplace.
About the Role
Key Responsibilities:
1. Consider whether the findings suggest that the case can be handled treated within your skill set or whether the individual requires an onward referral to another member of the MDT team or an external referral.
2. Where required, post initial assessment, undertake a more detailed assessment within your skill set.
3. Assist the individual in taking personal responsibility for the longer term and provide them with a plan of action, with scheduled follow ups.
4. Identify any obstacles to recovery including any psychosocial factors.
