Specialist Podiatrist (Band 7)
We are seeking an experienced and motivated Specialist Podiatrist (Band 7) to lead and coordinate podiatry services for hard‑to‑reach populations within Central London Community Healthcare NHS Trust (CLCH). This is a unique opportunity to deliver high quality, person‑centred podiatric care to individuals who face significant barriers to accessing healthcare. The post holder will act as a key link practitioner between the Homeless Health Service and Podiatry Service, supporting continuity of care and improving access to appropriate services.
The role involves autonomous assessment, diagnosis, and management of patients with a wide range of complex podiatric and medical conditions, including high‑risk and at‑risk foot care, as well as providing specialist advice, education and support to colleagues and the wider multidisciplinary team. Working in partnership with internal teams and external organisations – including community services, GPs, hostels and outreach centres – the post holder will contribute to reducing health inequalities and promoting inclusion within healthcare services. The post holder will also play a key role in safeguarding vulnerable adults and young people, working closely with social care and partner agencies to ensure safe and effective care, and will work flexibly across three London boroughs: Westminster, Kensington & Chelsea, and Hammersmith & Fulham.

* Work collaboratively with statutory and voluntary organisations to ensure continuity of care and improve access to services for hard‑to‑reach populations, promoting inclusion and reducing health inequalities.
* Deliver education and health promotion initiatives across the clinical network, raising awareness of foot health and general wellbeing.
* Contribute to the development and improvement of services, supporting pathways that enable access to appropriate mainstream and community services, ensuring a holistic approach to patient care.
* Undertake autonomous assessment, diagnosis and management of patients presenting with complex podiatric needs.
* Develop and implement individualised treatment plans, utilising a range of evidence‑based interventions for patients, including those in high‑risk and at‑risk categories.
* Provide specialist care and management for patients with complex needs, including those with diabetes, substance misuse issues, chronic neuropathy, long‑term conditions such as rheumatoid arthritis, wound care and tissue viability concerns, and HIV.
* Work in partnership with multidisciplinary teams, social care and external agencies to support safeguarding of vulnerable adults and young people, ensuring safe, effective and person‑centred care.
