Overview
The Intermediate Care Service offers care and therapy support to patients in the Cambridgeshire and Peterborough health system following an acute hospital admission, to step down from community hospital or prevent admission. The service runs seven days a week 0700 - 2200. The vision is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital as part of the HomeFirst model.
Responsibilities
* Provide leadership and management to the band 4 assistant care coordinators and band 2 and 3 integrated care workers within the Intermediate Care Locality, in alignment with the supervision structure.
* Ensure smooth implementation of tasks and responsibilities assigned to care coordination regarding allocations, care planning, referrals, pending list management and patient flow.
* Act as leader and manager to the care coordination team and enable collaborative working between care coordination, integrated care workers, and the therapy and nursing staff.
* Undertake appraisals, supervision, training and development of bands 2, 3 and 4 staff; assist the Senior Care Coordinator and Intermediate Care Team Lead to monitor and report on quality, productivity, and patient flow in line with internal KPI's.
* Visit patients in the community as required to support quality, patient flow and issue resolution related to delivery of care.
* Handle telephone complaints/enquiries and other communications with tact, diplomacy, and confidentiality in line with CPFT procedures and policies.
* Lead and manage recruitment in line with processes and policies for integrated care workers and band 4 care coordination staff.
* Support the Senior Care Coordinator in supervision, development and appraisal of assistant care coordination staff and integrated care workers. Travel around the county to meet strict time deadlines; hold a full UK driving licence.
Requirements
* Full UK driving licence and access to a vehicle; willingness to travel independently around the county
* Experience in care coordination, leadership or management roles
* Ability to work across multi-disciplinary teams and manage patient flow in community settings
* Willingness to undertake pre-employment checks (DBS where appropriate) and provide identity documentation as required
