Job Overview
This job overview is for a Band 6 Clinical Nurse Specialist or Allied Health Professional (AHP) specializing in respiratory care.

Role Summary

* Deliver specialist respiratory patient-centered care.
* Work within a multidisciplinary team (MDT) including Respiratory Consultants, Physiotherapists, Clinical Nurse Specialists, Advanced Clinical Practitioners, HCAs, and Therapy Support Workers.
* Collaborate with external professionals such as GPs, Urgent Care Teams, Community Nurses, and Palliative Care Specialists.

Key Responsibilities

* Provide specialist respiratory interventions.
* Support the MDT in assessing patients and developing holistic management plans.
* Assess patients with a variety of respiratory conditions, including complex physical and psychological needs.
* Develop and deliver individualized treatment programs.
* Understand and manage multiple lung conditions, such as COPD, Bronchiectasis, Interstitial Lung Disease, Asthma, and Oxygen therapy.
* Stay aware of local and national guidelines related to these conditions.

Main duties of the job

The successful candidates will provide specialist respiratory intervention, as well as supporting the multidisciplinary team in assessment and development of a holistic management plan. You will be expected to perform assessment of respiratory patients with diverse presentations and complex physical and psychological conditions. You will understand the assessment and management of multiple respiratory conditions and be aware of local and national guidelines for COPD, Bronchiectasis, Interstitial Lung Disease, Asthma and Oxygen.

The role is a predominantly Monday – Friday with the hours being 9-5, although there is a longer term plan to provide a 7 day service.

The role is available in Frimley Health; based in Farnham Community Hospital covering North-East Hampshire and Farnham and Surrey Heath.

Qualifications
Essential criteria

* Registered Nurse – Part 1,2,12 Or HCPC registered
* First degree in nursing/HCPC, or demonstrable experience at this level
* Teaching / assessment programme

Desirable criteria

* Diploma/ degree in respiratory care (COPD / Asthma) or willing to complete.

SPECIFIC COMPETENCIES

Essential criteria

* Demonstrates up to date knowledge and expertise in general nursing/physiotherapy
* Knowledge of key professional issues and NMC/HCPC guidelines and codes, relating to professional practice
* Awareness of community working and structures
* Knowledge of the COPD and asthma outcomes strategy, NICE & BTS guidelines
* Full driving licence and access to a car for use to travel across sites and perform domiciliary visits
* Able to work autonomously and within a team
* Demonstrates skilled written and verbal communication skills

Desirable criteria

* Advanced clinical assessment skills
* Experience of applying research evidence to clinical practice
* Presentation and teaching experience

Professional/ Specialist/ Functional Experience
Essential criteria

* Minimum 2 years relevant clinical experience in the same or related specialty OR At least 2yrs recent clinical experience in the acute setting
* Experience in managing the acutely unwell respiratory patient in the hospital setting
* Evidence of ability to work collaboratively

Desirable criteria

* CBG / ABG interpretation
* Previous community nursing experience

Special Requirements
Essential criteria

* Demonstrates a commitment to multidisciplinary working
* Able to be receptive to advise and guidance from senior colleagues
* Confidence, Patience and Empathy
* Demonstrates understanding of relevant national strategy / policy and how this relates to the service
* Willing and able to work weekends and unsocial hours
